Whats wrong with the user having to click on the attachment ?

If you are coding then why java ? Messing around with downloading plugins is unnecessarily complex for a simple one off task like this.

If you wanted to avoid java coding and plugin downloads you could email a link to a webpage (the form) which each user fills in and submits online. The act of pressing a submit button could send an email back to you with the details entered or could even store those details to wherever you wish to store them. The user would however have to click on the link in the email and you would have to write the html page that represents the form.
