This is a poem by Percy Ffrench, what you have written is part of the 1st & last verses.
There is the opening and then one verse relating to each of the provinces. 5 verses in total.
I suggest to check out Percy Ffrench collections in your local Library. Niall Tobin also does a version which is excellent
